Why does embargoed, impoverished Cuba have a life expectancy rate 2nd only to the USA?

How many Cubans have cars? boats? airplanes? swimming pools? drive race cars? have stressful jobs? have enough food to overeat? Lots of things besides health care that determine life expectancy.
